Insurance Claims
We accept most insurance plans, including medicare and medicaid.  Feel free to call the office to find out if we participate with your insurance provider.
 
Because there are now so many insurance companies with differing rules and coverage requirements, we must ask that each patient be responsible for knowing the particular requirements of his or her policy. We must depend upon you to advise us about such details. Our office policy brochures will give you detailed information about insurance filing, e.g., Blue Shield, Medicare, Medicaid, Medicare­ Medicaid combinations and Workman’s Compensation.
 
No Insurance?
We offer CareCredit or we can set up a flexible payment schedule option by simply inquiring when you call for an appointment.

If you have any questions for the nursing staff, call (859) 278-4960 Monday through Friday between 9:00 AM and 5:00 PM  Usually, our nurses can help you. If they have any concerns they will consult with a physician to confirm any questions.

If you have a question about your account, call (859) 277-9436, Monday through Friday between 9:00 AM and 5:00 PM and our business office will be happy to answer your questions.
 
PAYMENT POLICIES
 
Office Visits
Our fee for an office visit is due at the time of your visit.
 
We accept cash, check, Visa or Master Card. Upon request, we will provide you with the proper documentation needed to make it easy for you to get reimbursement from your health care plan.
 
Surgery Fees
You should know that our surgical fee includes six weeks of the usual post-operative care and follow-up visits.  As a courtesy, we are happy to assist you in the filing of most insurance claims and completing insurance forms and insurance precertification. You will be responsible for any and all balances not covered by your insurance. The ULTIMATE RESPONSIBILITY for the filling and processing of claims to satisfy your insurance carrier REMAINS WITH YOU. If you are unsure of any specific requirements of you insurance, PLEASE ASK. As the insured client, you are in the best position to follow up with your insurance carrier to ensure payment is being processed.
